Home Improvement and Support Team

Our Home Improvement and Support Team provide supported housing services for people with a wide range of needs to help them live more independently. This includes those with mental ill health, complex needs and disabilities.  

We also provide community support and advice through floating support or independent hospital discharge services. 

The support we provide depends on the needs of the individual but includes: 

  • Working with people who need extra help to manage their home and stay independent 
  • Helping people to manage money and pay bills 
  • Putting people in contact with other local services that can help 
  • Adapting someone’s home to make it more suitable to live in 

Services in Kent

Care Navigator Service

The Care Navigator team support patients leaving or at risk of being admitted to hospital to maintain their independence, increase confidence, safety, and security at home.  This service is free and impartial.  

Where we can help 

The team is located at the Queen Elizabeth the Queen Mother Hospital in Margate. 

Who we support 

Patients admitted or at risk of being admitted e.g., presenting to A&E, to the QEQM Hospital.

We can support you to make contact with services to help you maintain your independence, safety and security in your own home once discharged from hospital. We can help you to:

  • Find out about services available in your community
  • Improve your confidence in decision making and dealing with everyday problems
  • Help you take charge and manage your situation
  • Improve your sense of wellbeing

We accept self-referrals and referrals from Health and Social Care professionals. 

Health and Housing Service

The Health and Housing team work alongside the NHS Integrated Discharge Team and Rapid Transfer Services at a number of acute hospital sites in Kent.

The Home Improvement Agency service includes practical support, advice, adaptations, small handyperson jobs and information for older or disabled people to help them feel safe and independent in their home.

About the service

Our team can support customers to:

  • Make adaptations to your home to make it more accessible and easier to use
  • Apply for grants and loans, where available, from the local authority to pay for major works
  • Investigate self-funding options
  • Find insured and vetted building contractors to carry out works
  • Get advice on issues affecting your home such as rising damp or roof work
  • Get advice and information on a range of other services that might offer additional help within the local area
  • We also offer a handyperson service designed to help you with odd jobs and minor repairs, provided by our own DBS-checked staff at an affordable cost

Our advice is free, and we aim to eliminate the worry of rogue-traders and excessive fees for professional tradesmen.

We have highly experienced staff to help guide you through every stage of an adaptation or other major works to your home.

Our services normally operate Monday to Friday, 9am to 5pm.

How to make a referral

We accept referrals directly from customers or via other local organisations.

We aim to provide a response within 2 working days. If a home visit is required, we aim to visit within 10 working days.

Home Straight Service

The Home Straight team offers high quality, flexible and personalised support for people with hoarding behaviours.

About the service 

Our team deliver personalised support to customers in their homes aiming to address and reduce the risks connected to hoarding.  We work with customers to de-clutter homes enabling people to live more safely. We provide support to help create happier and healthier lifestyles, while improving physical and mental wellbeing.

We accept self-referrals and referrals from Health and Social Care professionals.

Supported Housing

In West Kent we offer a specialist supported housing scheme for people with learning disabilities or other support needs.

Where we can help

Our supported housing schemes are:

  • Frobisher Court, Dartford 
  • Sunhill Court, Pembury 
  • The Old Dairy, Tunbridge Wells 

We also provide support in the community for residents living in Paddock Wood, Pembury, Tonbridge and Tunbridge Wells.

Who we support

We support people with a diverse range of needs, including learning disability, autism, and mental health needs. Each person receives a different amount of support hours per week, assessed and funded by Kent County Council under the Supported Living contract. 

About the service

We provide a Supporting Independence Service (SIS) at The Old Dairy in Tunbridge Wells and at Sunhill Court in Pembury.

Through Live Well Kent we support people at Frobisher Court to improve their mental health and wellbeing. Live Well Kent is a free service delivered on behalf of Kent County Council and the NHS via two charities, Porchlight and Shaw Trust.

Our team also provide a Community Supporting Independence Service (CSIS) to people in their own homes who live in Paddock Wood, Pembury, Tonbridge and Tunbridge Wells.

Support is based around five key areas: 

  1. Achieve economic wellbeing 
  2. Enjoy and achieve 
  3. Being healthy 
  4. Staying safe  
  5. Make a positive contribution 

This can include prompting people to carry out daily care of themselves, teaching new skills, providing emotional support, and ensuring they are living a fulfilled life. We focus primarily on enabling and supporting people to live their lives as independently as they can.

Sanctuary Service

The Medway Home Improvement Agency works in partnership with Medway Council to provide advise and assistance with home security measures for people affected by domestic violence.

We install home security measures for people affected by domestic violence, which include:

  • Lock changes
  • Security chains/bolts
  • Door bars and grilles
  • Locking window handles
  • Cameras
  • Security lights
  • Personal alarms

Our aim is to help people feel more confident and safe within their homes.

Our services normally operate Monday to Friday 9am to 5pm, although the locksmith can carry out works in the evenings and at weekends.

How to make a referral

We accept referrals from professional bodies such as external domestic violence specialists, Independent Domestic Violence Advocates (IDVA’s), Social Services, police and other local authority departments.

Services in East Sussex

Housing Adaptations Support Service

The Housing Adaptations Support Service team provides support to residents requiring adaptations to help them feel safe and independent in their home.

Who we support 

Children and adults with physical disabilities and/or sensory impairment. The service is available to homeowners and tenants, with Landlord permission. 

About the service 

Our team can support customers to:  

  • Make adaptations to your home to make it more accessible and easier to use 
  • Apply for grants and loans from the local authority

We have highly experienced surveyors and support staff who can help guide you through every stage of an adaptation within your home from start to completion. 

Our aim is to ensure that we have provided the adaptation to fully meet your needs, so that you can live independently. 

Our service operates Monday to Friday, 9am to 5pm. 

How to make a referral

Please contact the Health and Social Care team (Tel. 0345 60 80 191) to arrange for an Occupational Therapist assessment.  

If an adaptation is required and the Local Council offer you a Disabled Facilities Grant, we will be contacted by them and asked to provide our adaptation service to you. The fees for our service are funded within the Grant.  

If you do not qualify for the funding following the financial means test, you can choose to pay privately for the service.
